Submit this to trading@sbz.com.zm along with the following:

1. Certified (stamped) Proof of Identity
   1. NRC
   2. Drivers License
   3. Passport
   4. Voters Card
   5. Birth Certificate (for minors)
2. Proof of Residence (must show your name and address)
   1. Bank statement (past 3 months)
   2. TPIN certificate
   3. Utility receipt (ZESCO, Lusaka Water and Sewerage or equivalent)
   4. Affidavit
   5. Tenancy/Lease agreement

For joint accounts, fill in the form with one individual's details but put both your names (as you see fit) on the "Full Names" section.

For "in trust of" accounts, put the trustee's details but fill in the "Full Names" section as "Guardian Names ITO Trustee's Names"
